Note: I made this tutorial under the assumption that anyone and everyone who chooses to utilize it, is Photoshop-stupid. This is for beginners and experts alike. So, enjoy. I only ask that you don't copy my icon exactly. Don't forget that you can mess around with the colours, the gradients, the brushes, and the angles for everything. Be creative with it all. Don't just take someone's tutorial and follow it exactly with your own image/text. Spice it up!
Porcelain Skin

001. Take your background picture and decide where you want it cropped.
002. Crop your background picture.
003. Duplicate your background picture.
004. Put on the filter Gaussian Blur at 3.0.
005. Duplicate your background picture again and drag it all the way to the top.
006. Change your blending mode to soft light at one hundred percent opacity.
007. Make a new fill layer with a gradient. The gradient is a standard, Photoshop gradient, called Steel Bar.
008. Set the gradient to 180-degrees; after you click okay, set the blending mode to soft light at one hundred percent.
009. Make a new fill layer with a solid, dark blue colour (any). I chose #0A193F.
010. Then set the blending mode to exclusion.
011. Add your text. (My text says "Lady Deathstrike.")
012. My font for "lady" was LainieDaySH, at 6pt, auto leading, zero tracking, and sharp anti-aliasing.
013. My font for "DEATHSTRIKE" was Times New Roman, capitalized, at 6pt, zero tracking, auto leading, and sharp anti-aliasing.
014. Add a simple border by creating a new layer. Select the entire selection, go to stroke, 1px white border, and ta-da!
Just in case you are unsure of some steps, here is a copy of what my layers pallet looked like at the very end of all of that stuff.
